Registration Forms

The following forms and information are available online for you to print and/or review. The Health Form, Child Profile, and Release and Waiver of Liability must be submitted at least two weeks before camp.

Please note: Some of these forms are to be printed and filled out. They are not designed nor intended to be filled out and submitted online.

If for any reason you are unable to open and/or print any of the forms, contact the camp and we will mail the required documents or attempt to send them as e-mail attachments.

Registration Form Print and submit one registration form per camper.

NEW! View/Make online payments on balances of registered campers. Available to online registrants and those who registered by mail. Valid email must be on file for mailed-in registrants. Contact us if you need assistance.

Health Form Required for all campers. Forms are retained on file so only one is required per summer. This form must be submitted at least two weeks before camp.

Release and Waiver of Liability Required for all campers. Forms are retained on file so only one is required per summer. This form must be submitted at least two weeks before camp.

Camper Profile Required for all campers. A useful tool that helps us get to know your camper. This form must be submitted at least two weeks before camp.

Camp Preparation Guide Basic resident camp information including what to bring (and what not to bring), drop-off and pick-up information, directions to camp, and FAQs.

Day Camp Information Basic Day Camp information including drop-off and pick-up times and locations, what-to-bring list, and schedule.

First Experience at Camp Information regarding a child's first experience away from home.

Additional Items Needed For Specialty Camps All the additional information and items needed for participants of the Adventure Camp, Challenge Camp, Wilderness Camp, Sports Camp, and Horse Camp.
